PAC Golf Simulator Liability Waiver
Acknowledgment of Risk and Release of Liability
By signing this waiver, I, the undersigned, acknowledge and agree to the following terms regarding my use of the PAC Golf Simulator:
I understand that the use of the PAC Golf Simulator and its associated equipment involves inherent risks, including but not limited to:
• Potential injuries from swinging golf clubs or other physical activities.
• Contact with equipment or other individuals.
• Damage to personal property or equipment.
I voluntarily choose to use the PAC Golf Simulator and assume all risks associated with its use, whether foreseen or unforeseen.
I certify that I am in good physical condition and able to participate in activities involving the simulator. I agree to follow all safety guidelines and instructions provided by PAC Golf staff.
In consideration of being allowed to use the PAC Golf Simulator, I hereby release, discharge, and hold harmless PAC Golf, its owners, employees, and affiliates from any and all claims, liabilities, or demands arising from injuries, damages, or losses incurred during or related to my use of the simulator. This includes, but is not limited to, claims of negligence or fault.
I accept responsibility for any damage caused to the simulator, its equipment, or the surrounding facility resulting from misuse or failure to adhere to guidelines. I agree to cover the costs of any necessary repairs or replacements.
I consent to the use of any photos or videos taken of me while using the simulator for marketing or promotional purposes by PAC Golf. [Optional checkbox for consent]
For participants under the age of 18, this waiver must be signed by a parent or legal guardian.
I HAVE READ AND FULLY UNDERSTAND THIS WAIVER. BY SIGNING BELOW, I ACKNOWLEDGE THAT I AM VOLUNTARILY PARTICIPATING IN THIS ACTIVITY AND ASSUME ALL RISKS ASSOCIATED WITH IT.
